$Referral requirements for stroke rehabilitation, physical rehabilitation, cardiac rehabilitation and speech therapy for stroke patients plus traumatic brain injury rehabilitation, occupational therapy, spinal cord injury rehab and hip fracture rehab.$ Referrals to HealthSouth Rehabilitation Hospitals of New Jersey at Tinton Falls and Toms River can be made by patients, physicians, hospital discharge planners, insurance representatives, employers, family, friends, private rehabilitation providers, case managers, and allied health professionals. A physician's prescription is required for services.
